Invention Disclosure Form

Patent - Invention Disclosure Forms

If you have a concept you would like patented, there are many ways of explaining to your Attorney what your concept is. This may include drawings, a written description, a model, a business plan, or even a verbal explanation. Depending on the concept to be patented, different forms of communication or disclosure may be appropriate. However, it is a good idea to reduce your concept to writing and record the date in order to show the date of conception of your patentable concept. The following forms are provided to aid in doing so.

Please do not disclose the filled out contents of these forms without a prior Engagement Agreement.
